Completing your initial patient forms prior to your first office visit will save you time, and make your first office visit that much easier.

Your comprehensive eye exam at Berlin Optical Expressions takes into account your overall health as we explore the health of your eyes, and work to improve your vision. Personal and family medical history, current and past medications, and other diagnostic information are important tools in establishing a holistic picture of your health, your risks for eye disease and related problems, and the best way to ensure you enjoy the best vision possible.

Log onto your Patient Portal from your computer or have one of our staff members help you fill them out when you get here. Allow yourself at least 15 minutes to do so before your appointment. The forms ask about your personal and family medical history, your current eye health and vision status, as well as other relevant information we need to best treat your eyes.
